In addition to the application and other forms provided here, additional documentation is required before funds can be disbursed.

- Documentation of Eligibility
- Copy of High School Diploma or GED
- Copy of acceptance letter from school
- Copy of class schedule to ensure at least part-time enrollment
- Copy of post-secondary school transcript to ensure passing grades
- Copy of financial aid package documenting all financial aid received and financial need
